When you visit our office for the first time, we will ask you to give us your medical and dental history as well as read a few of our policies. If you’d prefer, you may fill out these forms and submit them in advance through email or just print and bring with you to speed up your registration process. Please click on the links below.

Your first visit with Dr. Maria establishes a vital foundation.  During the first visit, we make sure to obtain important background information, like your dental and medical history, and give you time to get to know your doctor.  We want to know what your immediate and future concerns are with your oral health.  We will gather important clinical data, such as photographs, Xrays, head and neck exam, and a detailed exam inside your mouth. Most times, we will discuss your personalized treatment plan as well as financial information during this visit as well.  This allows us to provide you with the highest level of service with no surprises.

If you are interested in sedation dentistry and are apprehensive, we can also simply discuss concerns and leave the detailed exam to when you are asleep at a following visit.
